Medical Records/Health Information Management
Davis Regional Medical Center maintains medical records for patient care and legal purposes. Your medical record is a history of your hospitalization and treatment. We may use and disclose your medical information in connection with our healthcare operations, and we may disclose it to other providers who have a realtionship with you. In addition, you may give us written authorization to use your medical information or disclose it to anyone for any purpose.
We are required by applicable federal and state law to maintain the privacy of your medical information. We are also required to give you a copy of our notice about our privacy practices, our legal duties, and your rights concerning your medical infromation.
If you are concerned that your privacy rights have been violated, you may file a complaint with the hospital Privacy Officer. We will not retaliate in any way if you choose to file a complaint with us or with the U.S. Department of Health and Human Services.
